JOB DESCRITPION: Professional Caregiver
POSITION SUMMARY:
The Professional Caregiver provides companionship, personal care as needed and household assistance to clients in their own homes. The Professional Caregiver works under the supervision of the Hospice Supervisor and is assigned specific tasks through the hospice plan of care.
QUALIFICATIONS:
Education: Minimum high school diploma or equivalent.
Credentials: Eligible to work in USA.
Experience:
A minimum of one (1) year of verifiable work experience with a licensed home care or home health agency, adult family home , nursing home, hospital, hospice or private home. The exceptions to this requirement are education, professional certification or licensure, or other relevant life experience that may substitute for caregiver experience.
Core Competencies:
1. Fluent in both written and spoken English and ability to read, write and understand and follow directions provided for client care.
2. Able to speak more than one (1) language (preferred not required).
3. At least eighteen (18) years of age.
4. Able to pass a criminal background check.
5. Ability and willingness to provide care for people from various backgrounds without regard to age, race, religion, gender, veteran status,
national origin, culture, sexual orientation, marital status or disability.
6. Displays honesty and professionalism with high ethical standards.
7. Possesses maturity, flexibility, good judgment.
8. Demonstrates ability to work independently and as a collaborative team member.
9. Ability to communicate affectively and establish positive and productive relationships with office staff, clients, client representative and other
providers/professionals.
10. Practices good personal hygiene and dresses professionally.
11. Ability to perform all aspects of the job description with or without reasonable accommodations.
12. Successful completion of free orientation to Hospice policies and procedures.
RESPONSIBILITIES & FUNCTIONS:
1. Provides companionship to clients in their home.
2. Provides life enrichment opportunities for client such as reading to clients, listening/playing music/instruments, looking through photographs
and other memorabilia.
3. Performs household services essential to the client’s safety and care at home described in the plan of care including; laundry, meal preparation,
housekeeping.
4. Assists clients with personal care needs as described in the plan of care, which may include tasks such as bathing, toileting, dressing, transfers,
linen change, ambulation and positioning.
5. Perform only the tasks defined in the hospice plan of care for the client and observes, documents and reports any changes in the client’s
condition. May assist with medications as indicated in the hospice plan of care and agency policy.
6. Reports to the Hospice Supervisor any changes in the client’s mental or physical condition as well as changes in the client’s need for care.
7. Follows agency policies and procedures.
8. Meets the requirements for submission of time cards and documentation of assignments.
9. Respects the rights of clients, families, or client representatives to have different beliefs, opinions, cultures and customs.
10. Attends orientation and in-service training as required by the agency.
11. Works as a member of the home care team, keeps and maintains a professional attitude in the client’s home.
