Social Workers are core members of the hospice team and serve in providing emotional support, counseling to the patient, their family and friends. Everyone who is involved in the patients care or has a connection to the patient or family can take part in using our social services. Our social workers are here to help at your own time of need and at your discretion. Social services are available during a loved ones stay on hospice and after the program has stopped. Social Workers can offer assistance with advanced care planning, assisting in discussion about end of life care issues, assistance with supportive programs such as IHSS, meals on wheels, recommendations for grief support programs and with any other challenges you may be dealing with as a result of yourself or a loved one requiring hospice.
